
Chronic disease prevention and management in the North Country is getting a boost thanks to the Alice Hyde Medical Center (AHMC).
Alice Hyde recently made a generous gift to The Heart Network, sponsoring the Get Healthy North Country Community (GHNC) Integrated Health Network, a coalition of medical and behavioral health providers, public health units, area offices for the aging, social care agencies, and other interested organizations that collaborate to bring health and wellness support programs to the people that need them most. The network includes nearly 30 member organizations and a growing number of program hosts from across nine counties in northern New York.
“Alice Hyde’s generous donation supports education and resources that help North Country residents manage chronic medical conditions at home, reducing preventable hospital admissions,” said Dan Sweet, associate director of The Heart Network. “Their support of these important initiatives helps to make North Country communities healthier and improves people’s ability to stay well and practice healthy lifestyles. Alice Hyde’s partnership will help our network build upon the already excellent services available to those with chronic conditions, with the goal of expanding access, improving outcomes for patients and enhancing the sustainability of our region’s health care system.”
In announcing its support for Get Healthy North Country, Alice Hyde noted that The Heart Network is a “crucial partner focused on addressing health and wellness, including chronic and underlying conditions, throughout the communities we serve.”
“Ensuring residents of our communities have resources that facilitate early detection and education that supports at-home self-management are critical aspects of our shared mission of improving health and wellness, and clinical outcomes, for residents throughout the North Country,” said AHMC Hospital President Michelle LeBeau. “Our sponsorship will help support a variety of educational programs provided by the Get Healthy North Country team, including workshops on healthy living, chronic disease self-management, chronic pain and diabetes.”
The Heart Network and the GHNC Network administer the website gethealthynoco.org, which features a full slate of virtual and in-person workshops, offered at no cost to individuals and caregivers. These workshops cover a variety of chronic conditions, including diabetes, pain, cancer and more. Additionally, network partners work with healthcare providers to enroll patients, and train peer leaders who help lead workshops.
