The Get Healthy North Country Community Integrated Health Network and its partner organizations are recruiting and training Chronic Disease Self-Management Peer Leaders.
Peer Leaders are community members trained to help others understand how to recognize the signs and symptoms often associated with chronic conditions. These individuals work to empower others to take control of their health and live better, fuller lives.
From Jan. 4 to Feb. 15, the Health Initiative of St. Lawrence County — a founding member of the Get Healthy North Country Coalition — will host a Peer Leader training on Tuesdays and Thursdays, 9 a.m. to 11:30 a.m. The training is seven weeks long, with one session on Thursday during the first week, and two sessions each week on Tuesdays and Thursdays for the subsequent weeks. Peer Leaders should be friendly and open-minded, motivated by helping others from diverse backgrounds, comfortable speaking in group settings, willing to follow an evidence-based scripted manual, able to lead and/or facilitate with a co-leader, be dependable, and able to lead at least two workshops annually.

The Get Healthy North Country Community Integrated Health Network is a collaborative group of medical and behavioral healthcare providers, public health units, area offices of aging, social care agencies and other chronic disease prevention program stakeholders located in northeastern New York State.
