A free workshop series for people living with chronic health conditions is coming to Port Henry beginning May 28.
The chronic disease self-management program (CDSMP) workshop will be administered by The Prevention Team and hosted at St. Patrick’s Parish Hall, 12 St. Patrick’s Place in Port Henry, NY. The first session will be held Wednesday, May 28, from noon to 2:30 p.m. Subsequent workshops will be held noon to 2:30 pm. on May 30, June 4, June 11, June 13, and June 18.
This workshop is designed for anyone dealing with a chronic or limiting health condition. Participants learn self-management techniques and skills needed in the day to day management of any type of ongoing health concern. This program has also been proven to be effective for caregivers as well as friends and family members of those with a chronic condition.

This workshop is offered at no cost to individuals or caregivers. It’s one of many offered through the Get Healthy North Country Community (GHNC) Integrated Health Network, a coalition of medical and behavioral health providers, public health units, area offices for the aging, social care agencies, and other interested organizations that collaborate to bring health and wellness support programs to the people that need them most. The network is facilitated by The Heart Network and includes nearly 30 member organizations and a growing number of program hosts from across nine counties in northern New York.
